Medical Assistance - Emergency Details Form

Following the lead of Essex CCC, the Club is adopting an “Emergency Details” Form.  We are doing this because when a paramedic arrives, he needs to ascertain the medical facts about the patient as quickly as possible.  If the patient is not conscious, having this form to hand will save vital time .  

The “Emergency Details” Form provides basic information to help the Paramedic make the correct decisions about treatment.  Remember - it isn’t only the elderly who get problems – think of James Taylor and Patrice Mwamba!

The form can be found here and we would ask you to complete it and then keep it in a central position at the bottom of the main section of your cricket bag with the word “EMERGENCY” on show. Please keep it in a plastic bag so that it is safe from mud, damp, etc. as per the photo.

We are setting up this system on the basis that, if a player is ill or injured and cannot personally respond to questions raised by a paramedic, one of his team-mates can search his bag, find the form and thus provide vital information. Getting the form from the players bag should be done as soon as the emergency services are called so that the information is to hand when they arrive.  All Club members are receiving this email so everyone knows the procedure.  Please note that the medical information will remain private to each individual and will only used in an emergency.

The Club strongly recommended that you go along with this initiative so that it becomes a standard practice that we all work to - PLEASE DO IT NOW AND PUT IT IN YOUR CRICKET BAG.

We all hope the form will never be needed, but it is better to be prepared!
